Telangiectasia are not just “broken capillaries.” They are permanently dilated superficial blood vessels that remain open and visible to the skin’s surface. Their presence can be influenced by genetic predisposition, chronic sun exposure, hormonal changes, systemic vascular factors, and inflammatory conditions such as rosacea.

Health significance
Telangiectasia are not just simply cosmetic. They can indicate cumulative sun damage, impaired vascular regulation, or systemic health concerns including liver disease, connective tissue disorders, and long-standing hypertension. In rosacea, they are considered a diagnostic marker of vascular instability. Left untreated or unassessed, telangiectasia may progress, cluster, and further compromise skin barrier health, while also masking or delaying recognition of underlying systemic health concerns.

Mechanism of diathermy
Diathermy applies high-frequency electrical current through a fine probe, generating controlled thermal coagulation within the vessel wall. This results in collapse and closure of the vessel, which is then reabsorbed physiologically. Beyond surface, we always do consultations prior to treatment which highlights the importance of recognising, documenting, and monitoring vascular changes in the skin as part of a broader health assessment.

Not all tattoo removal is about regret.

Tattoo removal should never be performed by just anyone. We hear many places are offering this service now but doesn’t mean they should.

Because unfortunately it’s an unregulated area that we hope one day will change.

In the Bachelor of dermal sciences, students don’t perform this treatment until their 4th and final year. This is because of the skill, technique, knowledge and more that is needed to safely and effectively treat this concern.

Rosacea: A Chronic Inflammatory Disease

Rosacea is a chronic, immune-mediated skin condition involving neurovascular dysregulation, cutaneous inflammation, and epidermal barrier dysfunction.

Molecular Pathology
At the cellular level, Toll-like receptor 2 activation in keratinocytes leads to elevated kallikrein 5, which converts cathelicidin into LL-37—a pro-inflammatory peptide responsible for neutrophil recruitment, vasodilation, and angiogenesis.

Neurovascular Involvement
TRPV1 and TRPA1 channels on sensory neurons are upregulated, explaining why patients experience heightened flushing in response to stimuli like heat, alcohol, and spicy foods.

Microbiome and Demodex
Demodex folliculorum mites, along with bacterial antigens, contribute to inflammation and follicular disruption, particularly in papulopustular and ocular variants of rosacea.

IPL Mechanism of Action
Intense Pulsed Light, or IPL, delivers broad-spectrum wavelengths between 500 and 1200 nanometres. It targets oxyhemoglobin in superficial vasculature, causing vessel coagulation through selective photothermolysis.

IPL not only reduces erythema and telangiectasia, but modulates pro-inflammatory cytokines such as IL-1β and TNF-α, aiding in dermal remodeling and reduction of neurogenic inflammation.

Evidence-Based Practice
Multiple peer-reviewed studies confirm IPL’s efficacy in reducing persistent erythema and flushing, with significant results typically seen after 3 to 5 sessions.

Qualified dermal clinicians assess rosacea phenotype, conduct patch testing, and select parameters based on Fitzpatrick type and vessel depth, ensuring treatment safety and efficacy.

Postoperative scar revision and remodelling.

This client sustained a forearm fracture requiring surgical intervention. With his wound now fully closed and stable, we’ve initiated a microneedling protocol to support scar remodeling.

Micro needling, also known as percutaneous collagen induction, is supported by recent studies (Sheck et al., 2022; Karkour et al., 2024) to improve surgical scar appearance when introduced early in healing—typically from 6–8 weeks post-op under professional guidance.
